RED BANK – Visiting Nurse Association Health Group as announced that Wen Dombrowski, M.D. is joining the organization as the new medical information officer and vice president, Connected Health.
Dombrowski brings years of experience in health care and technology to the organization. She is a physician leader in informatics, social media, home care, community health and care coordination. She has developed technology, business and health care solutions to improve the quality of life for those with complex medical and social needs.
Her skills and experience will greatly assist VNA Health Group in achieving its mission of keeping the most vulnerable people in their communities and at home.
Dombrowski will serve as the physician leader for VNA Health Group’s mobile and digital technology, care innovative and quality improvement initiatives.
Dombrowski previously served as the director of clinical informatics at Morris Heights Health Center where she oversaw clinical data initiatives at a multispecialty, multisite network of community health centers in underserved communities.
Prior to that, Dombrowski served as assistant medical director of VSNY CHOICE Health Plans where she advised interdisciplinary teams on complex care management for the highest risk, elderly, and disabled Medicare and Medicaid members to support their home-based care and prevent rehospitalization. She also has advised other health care provider organizations, technology companies and industry organizations as a consultant regarding health care transformation, technology, social media and senior services.
Clinically she is a practicing physician who has cared for patients across the continuum of care including outpatient, hospital, post-acute and long-term care settings; most recently she has been making house calls to low-income patients who have many prior hospitalizations or chronic severe illness. She also has an extensive history of volunteerism helping frail elders, individuals with developmental disabilities and the poor.
Dombrowski is board certified in internal medicine and geriatric medicine. She completed her internal medicine residency at St. Luke’s-Roosevelt Hospital Center, an affiliate of Columbia University in New York. She completed her geriatric medicine fellowship at Mount Sinai Medical Center. She earned her Bachelor of Science in cellular and molecular biology and minored in philosophy from the University of Michigan, her doctorate of medicine with a concentration in ethics and humanities from the University of Rochester School of Medicine and received her Master of Business Administration from the IE Business School in Madrid.
